Smile Plan Terms

Effective date: 21 July 2026

This Smile Plan Membership Agreement sets out the terms of the Smile Plan membership provided by Dr Phil Aesthetics, a trading name of Phil End Dentistry Ltd ("we", "our", "us").

Smile Plan benefits

The Smile Plan is designed to help patients maintain excellent oral health through regular preventive care and exclusive treatment discounts.

For a monthly fee of £20.00, members will receive:

• Two preventive care appointments per year, including:
  • Comprehensive full-mouth dental examination
  • Professional hygiene appointment
  • Composite restoration polish (where applicable)
• Two whitening gel syringes per year
• 20% discount on all other dental treatments carried out at the practice (excluding specialist referrals and laboratory fees where applicable)

Membership commitment

By joining the Smile Plan, you understand and agree that:

• The Smile Plan requires a minimum commitment period of 12 months.
• Monthly payments of £20.00 will be collected by the practice using the agreed payment method.
• The total minimum commitment over the initial 12-month period is £240.00.
• Benefits are available only while membership payments remain up to date.
• Missed appointments may be subject to the practice's cancellation policy.
• The 20% treatment discount cannot be used in conjunction with any other offer, promotion, or discount.
• If membership is cancelled before the completion of the 12-month minimum term, the practice reserves the right to recover the value of benefits already used that exceed the payments made.
• Following the initial 12-month term, membership will continue on a monthly rolling basis unless cancelled by either party with 30 days' notice.

Patient declaration

By joining the Smile Plan, you confirm that you have read and understood the terms of this Smile Plan Membership Agreement. You agree to the monthly payment of £20.00 and understand that you are committing to a minimum membership period of 12 months.
